The “person in charge” at the restaurant was unable to demonstrate knowledge of food allergy guidelines and couldn’t sufficiently describe employee heath rules and requirements, the report says. Employees drank from unapproved cups and water bottles and stored them above boxes of cheese, according to the report.

Unwashed mushrooms were stored above ready-to-eat foods in a walk-in cooler, and food was encrusted on pans that were marked as clean.
